Do I have a Deductible?
Yes. It is $300 per person for in network doctors and $400 per
person for out of network doctors. See the
Schedule of Benefits for more
information about deductibles.
Can I get my Diabetic
Supplies by mail-order?
There are in-network providers of diabetic supplies - call
HealthLink.
Can I get my self-injectables
in network?
There are in-network providers of self-injectables - call
Freedom Network or HealthLink.
Is infertility treatment
covered?
Neither the treatment or diagnosis of infertility is covered. See the
Medical Exclusions section on this
website for more information.
How do I contact my
prescription company?
Call
Caremark at 1-800-966-5772.
Are private rooms covered
for inpatient stays?
Only semi-private rooms are covered for an inpatient stay.
Is massage therapy covered?
Massage therapy is not covered even if it is part of a physical therapy
program.
How do I appeal a claim
payment?

Is there a LASIK benefit?
LASIK has a lifetime benefit of $1600 per eye, paid out after a $25.00
deductible per eye. For more information, see the
Vision Benefits section of this
website.
What is an out of pocket
maximum?
You have an out of pocket maximum of $1,500 per person for in network
services and an OOP maximum of $2,500 per person for out of network.
This is the total amount of co-insurance you, the member, will pay in
one calendar year before the plan pays at 100%.
If I am retired, do I have
vision benefits?
Retirees do not have routine vision benefits. However, certain
medical conditions could be covered under the medical Plan.
If I am retired, do I have
dental benefits?
Retirees do not have dental benefits.
